TERRENCE “TERRY” KOZAK

Terrence "Terry" Kozak, 58, of Hayesville died Sunday (Oct. 9, 2005) at his residence. A native of Cook County, IL, he had been living in Hayesville for a year. He was the son of the late Walter and Wanda Karrweck Kozak. He was a veteran of the Vietnam War, serving in the US Navy. He had worked for 38 years at Jewel Food Store in Chicago as a meat cutter. Terry was of the Catholic faith.

Surviving are his wfe, Bonnie Smith Kozak; two sons, Jerry N. Criscione and wife, Tammie and Anthony M. Criscione and wife, Mary all of Chicago; a brother, Kenneth Kozak of San Francisco, CA; and seven grandchildren.

Funeral Mass at 10:30 AM Thursday (Oct. 13) in the Immaculate Heart of Mary Catholic Church with Father George Kloster officiating.

The family will receive friends from 6-8 Wednesday evening at the Ivie Funeral Home, Hayesville.
